Upon lottery notification, parents will be given the necessary information to register. This will include but is not limited to:

• Registration dates
• Enrollment requirements
• Electronic registration forms
• Uniform supplier information
• Supply lists

Please note, the following items are required for enrollment:

• Birth certificate
• Physical record
• Immunization record
