Administration of Medication Form & Guidelines
I understand that medication provided to the school for the administration to my child must be checked in by the parent/guardian to the front office or clinic staff (not your child's teacher). This means that I am aware that my child/ren MAY NOT TRANSPORT MEDICATION TO AND FROM SCHOOL. THIS IS AN EXPELLABLE OFFENSE!!!
